If you have a patient/patients to refer, please email isc@mendingkids.org with the following details:

• Patient name, location, and age;
• A general photo of the patient and a photo to indicate the condition(if applicable);
• Medical reports, diagnosis reports, and/or doctor's notes;​​
• If you are referring a cardiac patient, please include an echocardiogram performed within the last 6 months;

If a doctor's diagnosis can not be acquired, please describe the patient's condition as detailed as possible so we can send it to our medical consultant for help.
Once you sent over the requested documents, please allow us 3-5 workdays to get back to you regarding whether we can help or what we can do for the next steps. 

Unfortunately, we can not help with cancer, organ transplant, or patients with down syndrome, but we can send you a list of other amazing nonprofits that target these specific conditions.
